01Where are you?
We are at 1130 NW Johnson Street, Suite 300, in Portland's Pearl District. Street parking and nearby garages are available, and several bus and streetcar lines stop close by.
02How do I prepare for my first visit?
Bring a photo ID, your insurance card if you have one and a list of your medicines. Health history forms are completed in our private patient portal or on paper at the office, not on this website.
03What if I need to cancel or reschedule?
Please give us at least 24 hours' notice by phone so we can offer your time to another patient.
